Support at Home, in plain English
A calm starting point for understanding assessments, plans and next steps.
Support at Home can feel difficult to understand when you are trying to make decisions for yourself or someone you love. The first step is usually an assessment of needs, goals and the support that may help at home.
Once an outcome is available, the support plan can guide conversations with a registered provider. Services may include help with everyday routines, personal care, transport, respite or other approved supports.
You do not need to work everything out in one conversation. Write down the routines that have become harder, the goals that matter most and the questions you want to ask.
