Prepare for an assessment
A simple checklist of everyday details and questions to gather before a care conversation or assessment.
An assessment is a chance to describe how everyday life is going, not a test that your family needs to pass. Think about the routines that feel easy, the tasks that take more energy and the moments when extra support would make a difference.
It can help to bring a short list of medications, appointments, mobility changes and regular activities. Notes from family members or a trusted support person can also add useful context, especially when needs change from day to day.
Before the conversation, choose a few priorities for the coming months. These might include staying safe at home, continuing a favourite outing, preparing meals more confidently or giving a family carer time to rest.
